Our Community Promise – A promise backed by purpose

At the Foundation, we believe where you live should never stand in the way of having the care and support you need to thrive.

Our Community Promise is our commitment to every person and community we work alongside:

  • Your Community — We listen first and shape what we do around what matters locally.

  • Close to Home — We bring care and support closer to the people who need it.

  • More Than Health — We look beyond healthcare to what helps people and communities thrive.

  • Together — We work with communities and partners to create meaningful, lasting change.

It’s our purpose in practice — and the standard we hold ourselves to.
