HealthAccess Home Care is a social enterprise of the Healthy Communities Foundation Australia.  Our virtual care solutions work with primary health networks, hospitals, community services, NGOs and other organisations to support improved access to low acuity care in people’s homes.

HealthAccess Home Care is designed around the needs of people in their homes - whether that is a residential home, group home or aged care facility.  

 

We tailor solutions to the unique needs of our customers to ensure that the type and scope of care is aligned to the needs of our consumers.  That could be a fully virtual direct to consumer after-hours GP services, a hybrid GP/Nurse Practitioner service delivered on-site and virtually in a residential aged care facility or a group home.  

Services delivered by HealthAccess Home Care differ from those delivered by HealthAccess Virtual Care.  Home Care services are delivered directly to consumers weigh funding support from a PHN, NGO or other agency.  Virtual Care services are delivered to an institution which retains the primary therapeutic relationship with the customer and HealthAccess supports that relationship.

Residential Aged Care

HealthAccess Home Care is a service available to residential aged care facilities across Australia.  We support your staff, your regular GPs and residents to minimise preventable illness, unnecessary transfers and avoidable hospitalisations whenever your regular GP is not available - 24/7, 365 days per year! 

HealthAccess Home Care works with residents and staff to provide GPs and Nurse Practitioners with experience working in residential aged care to assist residents with telephone and video consultation via tablet or laptop. A hybrid model of care, we understand the importance of a schedule of on-site visits and continuity of care from regular GP and Nurse practitioner to build therapeutic trust and deliver better quality health outcomes for residents.

In-Home & After-Hours

HealthAccess Virtual Care can deliver a dedicated After Hours and in-Home GP Helpline that supports your patients to access healthcare when they need it, while ensuring that you maintain continuity of care.

 

We support communities and local GPs/Visiting Medical Officers by providing patients with a 24/7, 365 day per year after hours Telehealth service, treating low-acuity presentations quickly and effectively - so patients don’t have to go to the hospital. 

 

Low acuity treatments supported by the service include:
