My Care delivers a hybrid face-to-face and virtual model of care
My Care is a residential aged care service delivered by The Healthy Communities Foundation Australia (THCFA), offering a unique hybrid face-to-face and virtual model of care.
We deliver high-quality, resident-centred clinical services that prioritise wellbeing, safety, and timely access to care. Our model is designed to reduce avoidable hospital transfers, strengthen on-site clinical governance, and provide evidence-based medical care within the residence.
My Care brings together an experienced Executive General Manager, General Practitioners, Nurse Practitioners, Registered Nurses, and dedicated administrative support to deliver a structured, responsive, and evidence-informed model of care.
Our integrated virtual care capability ensures residents receive prompt assessment and treatment, minimising delays and disruption.
Core Services
Quarterly face to face and weekly virtual clinics
Rapid-response virtual reviews for deterioration or acute needs
On agreed scheduled, clinical rounds - with the same, dedicated My Care GP and/or Nurse Practitioner
After hours, weekend and public holiday clinical support is available for your residents.
Chronic disease management planning and monitoring
Allied Health referral coordination if required
Medication management and review coordination
After-hours support via Nurse Practitioner for urgent matters
Palliative and end-of-life support
Care planning and documentation management
Mental health & dementia assessment pathways
