We partner with facilities across Australia to deliver consistent, high-quality medical care that reflects the unique needs of its residents. Whether you require care for a single resident or a full facility, we can design a care model that works for you.

Each resident is supported by a dedicated GP working alongside an experienced Nurse Practitioner. What sets our service apart is our flexible hybrid model, combining face-to-face visits with virtual consultations via video and phone. This ensures care is timely, accessible, and tailored to each resident’s individual circumstances.

What we offer

We deliver high- quality medical care for residents with a range of chronic and complex needs, including those:

living with chronic conditions requiring regular review

experiencing mobility or cognitive barriers to in-person care

at high risk of preventable hospitalisation

receiving palliative care

living with dementia or mental health concerns.

Our service is collaborative and integrated. We work closely with residents, families, and facility staff to deliver care that supports dignity, wellbeing, and independence.

We also take a bespoke approach to service design. We’ll work with your facility to tailor a model that reflects your resident mix and operational needs. This may include coordination with local allied health professionals such as physiotherapists, podiatrists, and dietitians.

Our clinical approach

What we offer:​

GPs provide virtual and in-person clinical care, prescribe medications using accredited software, and manage referrals to third-party services such as pathology, radiology, and specialist providers.

Nurse Practitioners offer after-hours support for urgent and palliative care needs and provide assistance and support to GPs during clinics.

Our team participates in monthly management meetings, Medication Advisory Committee meetings, and other clinical governance forums to ensure integrated and transparent service delivery.

The service is overseen by a highly experienced aged care Registered Nurse and former facility manager, with deep knowledge of the Aged Care Quality and Safety Standards, SIRS reporting, and an extensive clinical background.
Billing
All clinical services are bulk billed. A facility service fee applies, based on the number of beds, to cover coordination and ongoing service delivery.
